RENTAL AGREEMENT

These are the forms that are needed for Children under the age of 17, signed by their parents, giving their permission for the child to play and need medical information. Also Children under the age of 17 also must have a parent sign the release of liability located below.

PARENTAL CONSENT

Everyone needs to sign the release of liability.
